FULL-TIME PRIMARY CARE- MACON, GA ***Position SummaryCVS Health is
seeking a Nurse Practitioner (NP) to join our Primary Care team. In
this capacity, you will work in collaboration with a dedicated
clinical care team to provide evidenced-based care to a panel of
patients. The clinical care team will work with the provider daily
to drive better outcomes for entire panel of patients. -This will
include accurate assessment, diagnosis, treatment, management of
health problems, health counseling, and disposition planning. This
role will report to the practice manager.PRIMARY DUTIES AND
RESPONSIBILITIES: Responsibilities include but are not limited
toPatient-Centered Quality and Safety
- Uses knowledge of etiologies, risk factors, pathophysiology,
presentation, and epidemiology of medical conditions relevant to
primary, acute, chronic, and transitional to provide care to
patients
- Performs history and physical assessments of patients that are
accurate, complete, concise, relevant, and appropriate for age,
gender, and clinical symptoms.
- Provides patient counseling; inclusive of pregnancy prevention,
STI Prevention/safer sex practices, contraceptive care counseling
and medication management
- Counsels and educates patients and families on health
maintenance and respond to patient care inquiries
- Evaluates and treats patients' medical needs with safe and
high-quality treatment
- Documents all patient care within an electronic health record
in accordance with current policies and procedures
- Works as a member of the clinical care team providing care and
coordination of our patients with internal and external colleagues
ensuring the highest level of care is always provided for all
patients
- Provides patient-centered care that is compassionate,
appropriate, and effective for the promotion of health, prevention
of illness and treatment of disease
- Refers to appropriate specialists and patient care components
as appropriate
- Maintain patient confidentiality in accordance with PHI and
HIPAA standardsCommunication and Interdisciplinary Collaboration
- Collaborates with clinical care team to ensure the delivery of
high quality, patient centric, longitudinal care
- Participates in and in some cases leads clinical care team
huddles
- Demonstrates core values of CVS Health in all communications
and interactions
- Communicates effectively with members of the clinical care
team, other health professionals and health-related agencies.
- On call rotation shared with providers in market for after
hours patient clinical needs or shift coverage

- Assist with hiring, development, and evaluation of Primary Care

- Complete necessary tasks for clinic operations, including but
not limited to following up on lab results, and receiving incoming

the providerRequired Qualifications
- A minimum of 1 year experience as a Nurse Practitioner is
required. -Experience in Family medicine or primary care practice
preferred. -
- Internal applicants with less than 1 year experience are
eligible with successful completion of the CVS Minute Clinic
Primary Care Training Program
- Active Advance Practice Registered Nurse, Nurse Practitioner
license in good standing
- Current National Board Certification in Family Practice
- ADDITIONAL REQUIRED SKILLS AND ABILITIES: -

the management of chronic disease conditions.

- Completion of master's degree level Family Nurse Practitioner
with current National Board Certification and State of Employment
license to practice in the Advanced Practice role required.Pay
RangeThe typical pay range for this role is:$37.66 - $81.11This pay
range represents the base hourly rate or base annual full-time
salary for all positions in the job grade within which this
position falls. -The actual base salary offer will depend on a
variety of factors including experience, education, geography and
other relevant factors. -This position is eligible for a CVS Health
bonus, commission or short-term incentive program in addition to
the base pay range listed above. - -In addition to your
